How can I obtain copies of my Medical Records?

Contact our copy clerk at (208) 746-1383 x6411 or click here for instructions on how to download and fill-out the Medical Records Release form. The release form can be filled out in person, faxed to you to be filled out, or mailed to you to be filled out. The release form should be mailed or faxed back to Valley Medical Center at: Valley Medical Center, Medical Records, 2315 8th St., Lewiston, ID 83501 or faxed to (208) 746-6348. There may be a fee depending on your situation.
